Hi Can anyone help me with a format that we can use if employee is on PIP to track his performance

Here's what we have at Springworks. Also each PIP is updated on Keka as well.

A very simplistic one that I use on google sheet (easy to collaborate). You can add comment column as well. PIP letter has to go alongside this sheet. @Rohan Mankad's document looks good too!
