Where I work, we use Pingdom for availability monitoring (with a smattering of their RUM and Synthetics added), and AppDynamics for performance monitoring (thought about newrelic, but we got a better deal from AppD- they were fattening themselves up for sale at the time, so we got good terms, IIRC).
