Does spending more that I make mean in terms of current expenses or does it also include budgeted savings outside the budget (I know that doesn't make sense really - to have savings without money to actually save, but...)

Case by case, but if you can't reasonably save for your goals like a reasonable retirement or reasonable simchas, then absolutely.
