If I remember correctly Use Client IP requires USIP enabled.

That is not enabled

Is IIS configured to log the X-Forwarded-For header?
n
Not sure, will verify 👍

some apps also don’t use x-forwarded-for they call it something different so you need to make sure you pass the header it expects to see
like Forwarded or x-forwarded-ip for webmail

Isn't it that with "Client IP / X-Forwarded-For" the actual client IP is sent in an HTTP header, but if we enable USIP and "Use Client IP" the request appears to originate from the client IP and this would result in the server responding directly to the client, rather than piping it through the load balancer?

so, it turns out that it was working correctly after all and there was a miscommunication, so the settings as pictured above + the advanced IIS logging settings is working as expected.
Thanks for the input everyone!
