Are you using ControlUp? Maybe it's possible to set up af trigger, when more than two failed connection has occurred, set the VDA in maintenance mode and then look into the problem.

I second what Daniel proposes. With ControlUp its easy-peasy (except if you want to fire something if the event happend after x times, thats tricky)... If you dont have ControlUp you can create an scheduled task, which kicks a powershell script when a specific Event occurs and puts the machine in maintenance mode. The task have to run as a user with admin rights on CVAD though.

"The task have to run as a user with admin rights on CVAD though." Doesn't need to be full admin rights though. Can be a user with only rights to set MM mode really. I do this for my install and maintenance scripts. Very limited user account.
@Arthur I'm interested in more details about what you are seeing in the failures though. I have been slow rolling CU3 and haven't seen anything like what you are describing that I have been able to document. Have seen servers go into an "Agent Error" mode, that requires a restart to fix. The VDA thinks it is still registered, but Studio doesn't so users can't connect to it. Delivery Controllers are CU2 still though so maybe something in the way things communicate.

@Dennis Parker In my opinion it's not related to CU3 - it's just a lack of "proper" health check of Citrix components when the OS is no longer working properly and Citrix is still trying to redirect users to "registered" servers, but such servers are not able to handle new users.

Citrix performance analytics has black hole machine insights which is pretty handy to identify machines which are registered but may be having issues and have a large number of connection failures

@Arthur Thanks for the clarification. I see what you are saying now. We do occasionally have similar issues there, but not too often.
