Ffs... I think I switched the authentication method by mistake when looking through the settings earlier in the day and that's why everything was broken. Switch back to github app auth and now everything seems fine

Ah yeah...funny you mention this because I'm currently working on making our onboarding process more robust (including what happens when you're kicked out due to auth issues)
